Regional Teams

Find and contact the After Stroke team serving your province

Our regional teams support the delivery of After Stroke across the country.

Each of our teams works together to develop and grow partnerships with community organizations, hospitals and rehabilitation centres to support stroke survivors and their families. These teams are supported by stroke survivors and caregivers who give their time as volunteers to support the delivery of peer support programs and After Stroke services.

Even if you do not live in one of the areas below, you can still reach out to After Stroke to access tools, resources and referrals.
